IDD Intake Referral Coord.

Must have bachelors degree in human service field.

This position has five areas of responsibility: 1) to serve as the single point of entry for IDD services; 2) to assist with SSLC applications for individuals with intellectual and developmental disabilities; 3) to complete and update permanency plans for consumers under the age of 22 who live in a group home setting; 4) to maintain current information about local resources; and 5) to promote consumer choice by presenting options in various settings.
