HCS Service Coordinator

Must have a bachelors degree in the human service field.

The main purpose of a Service Coordinator is to support and assist people with intellectual and developmental disabilities in achieving their personal goals. The service coordinator assists each consumer, along with the Program Provider, in accessing medical, social, educational and other appropriate services and supports that will help that individual achieve a quality of life and community participation that is acceptable to the individual. This is accomplished through: 1) assessment, 2) service planning and coordination, 3) monitoring, and 4) crisis prevention and management.
