Performs abstraction of clinical information for CMS and Payers Coordinates aggregation of data from within and outside the department Attends and participates on assigned performance improvement committees and team meetings Ability to exercise judgment, courtesy, and tact in receiving callers, in giving information and obtaining information, and in making proper disposition of problems Assists in preparing monthly, bi-monthly, quarterly, and annually quality reports as assigned Abstracts clinical data and entry to clinical abstraction software that meet CMS Core Measure performance improvement initiatives Create and maintain patient registries for an assigned panel of patients with chronic diseases, preventive health needs and other registries as needed Using data from registries, identify gaps in care for patients with identified diagnosis, those identified as high risk and those with outstanding preventive health needs while documenting contacts, education, and appointments provided...Coordinator, Quality, Technology, Healthcare, Clinical
